Dr. Smith Retirement

After 36 years of medical practice with Associates in Family Medicine, Dr. Jerome Smith retired from AFM July 31 , 2011. Dr. Smith hosted a gathering open to all his patients and AFM thanks all of his loyal patients who attended to wish him well. Associates in Family Medicine was incredibly fortunate to have had Dr. Smith’s service for so many years. He served as the “gold standard” for family physicians in Fort Collins. Dr. Smith delivered hundreds of babies at Poudre Valley Hospital and provided personalized family centered care for several thousand patients in Fort Collins. Dr. Smith is a former winner of the Colorado Academy of Family Practice Physician of the Year Award and served on countless committees over the years. He has been instrumental in the success and expansion of AFM. We wish him the very best in his retirement.

AFM is pleased to announce that Dr. John Cawley, MD, joined Associates in Family Medicine at our Timberline location. Dr. Cawley obtained a BA in Biology from Dakota Wesleyan University in South Dakota and attended Medical School at The University of North Dakota School of Medicine. He is completing his Family Medicine Residency at the Fort Collins Family Medicine Residency program in the Poudre Valley Health System.

Much like Dr. Smith, Dr. Cawley has medical interests that span the full spectrum of family medicine, from pregnancy care to senior care. He is accepting new patients and assuming care for Dr. Smith’s existing patients. Please call 229-9800 to schedule an appointment.

Although Dr. Smith is truly impossible to replace, Dr. Cawley and the AFM care team continue to provide Dr. Smith’s patients with excellent, compassionate care.
